Group Health

Provides coverage for injury and sickness for your employees and their dependents. Traditional health programs and Health Savings Account programs are available. Group health insurance is very expensive, and careful planning is very important to be sure that you are receiving the best available benefits within your budget. Individual / Family Health

Provides coverage for injury and sickness for yourself and/or members of your immediate family. Similar to Group Health, Traditional and Health Savings programs are available to individuals and families, but they are often subject to stricter pre-existing condition limitations and limited benefits for certain conditions, i.e. pregnancy. When available, Group Health benefits and/or COBRA benefits are preferable to Individual / Family coverage from a coverage standpoint, but Individual / Family coverage may be less expensive.

Life

Provides a limited death benefit to your employees with options for their purchase of additional insurance for themselves and/or their dependents through convenient payroll deduction. For most Americans, the life insurance coverage that they receive at work is the only coverage they have.

Disability

Provides paycheck protection, should the employee be unable to work for a period of time due to an illness or injury. Short-Term Disability and Long-Term Disability policies are available on either an employer-paid or employee-paid (voluntary) basis.

Long Term Care

Provides benefits for skilled long-term health care in either a nursing home or residential setting. Claims are based on the need for assistance with 2 out of 6 activities of daily living or cognitive impairment. This benefit is available as a carve-out program for management employees or as a total group benefit with a base employer-paid benefit and optional buy-up benefits on a voluntary basis.

Dental

Provides benefits for routine and restorative dental care for employees and dependents. Programs are available that use network dentists or that will provide coverage with any dentist.

They provide all dental specialties - pediatric, general, periodontal, endodontic, orthodontic and all other dental services - in one location. The Smilebuilderz discount program is available to groups who want to help their employees to manage the cost of dental care without the premiums associated with a traditional dental plan. Vision

Provides benefits for routine vision exams and hardware to correct vision, i.e. glasses or contact lenses. Programs are available that use network vision providers or that will provide coverage with any provider.
